Provide specialized support to individuals with developmental disabilities. Must complete DDD-specific training and meet state requirements.
High school diploma or equivalent
DDD training certification (or willingness to obtain)
Experience with developmental disabilities preferred
Clean background check required
Valid driver's license
Deliver in-home skilled nursing care, medication management, and care coordination under physician-directed care plans.
Active RN license in New Jersey
1+ year clinical nursing experience
OASIS documentation familiarity preferred
Reliable transportation for home visits
Strong communication and assessment skills
Assist clients with daily living tasks, personal hygiene, and companionship while following individualized care plans.
Active CHHA certification
Compassionate approach to client care
Ability to assist with ADLs safely
Weekend availability is a plus
Current CPR certification preferred
Coordinate multidisciplinary services, schedule follow-ups, and ensure documentation remains compliant with agency and payer standards.
2+ years healthcare coordination experience
Knowledge of Medicaid/MCO workflows
Strong organizational and communication skills
Experience with EMR systems
Ability to manage high-volume caseloads
